Teams are asked to checkin by providing the documents (listed below) via email to by Monday, August 19, 2024.
  • ID cards for players, coaches and team managers.
  • Official state roster with any non-participating players crossed out.  If a player is not on the official state roster (i.e., guest or club pass player), please write in the player's name, jersey number, and DOB at the bottom of the official stamped team roster.  
  • Completed Team Acknowledgment Form signed by a club official.
  • Completed Team Emergency Contact Form.
  • Permission to travel from your state association, if applicable.  
Please note that original, laminated ID cards must be presented to the game official prior to the start of each match.  Please have medical releases available at matches, if needed.
